Referral

A medical referral is not necessary to access our service. Our services may be claimed through some health funds (depending on your level of cover).

You will require a Medical referral if:

1. You are claiming through Medicare
(check eligibility with your doctor)

2. You are a Veteran’s Affairs Gold Card Holder.
(Note DVA does not cover high care residents)

3. You are attending our MBS clinic.

To make a referral to ANY of our locations, please complete our Client Intake Form. Alternatively fax referrals to 99791296 or email them to info@mlsp.com.au.
